How to Report an Incident

When reporting a lost or stolen piece of equipment, please follow these steps:

  • Determine if the equipment is lost or stolen.
  • Complete the Lost Stolen Technology Asset form (PDF file) located in the Attachments section of this page. 
    • If stolen, you must file a police report with local jurisdiction and provide the police report number on the Lost Stolen Technology Asset form.
  • When the form is completed please email the completed Lost Stolen Technology Asset form (PDF file) to TechSupport@sjcd.edu.
  • Check your email or client portal for related status updates.
