The weblinks for most Forms used at San Jac should start with "federation.ngwebsolutions.com".

If yes, please use the following:
Students' username is (LastName).(FirstInitial)(LastSixG#)@stu.sanjac.edu. Example: Garcia.J123456@stu.sanjac.edu. 
Employees' username is firstname.lastname@sjcd.edu
Your password is the same as SOAR, SOS, My San Jac Portal, or SJC Email.

If you are still having trouble logging in, you can also try resetting your password and then try the form again. Go to https://sanjac.edu/password.
The link should start with federation.ngwebsolutions.com.
If someone at San Jac provided the link to the form, please list the name/email address of the person who provided you with this link.
